Please find the following link Environmental Protection Agency Act, 1992 to the appropriate forms to use in the event of taking a noise complaint to court. The first form under section 108(3) is the form to used to make a complaint to the district court (this form is obtained from the District Court). The second form under section 108 (1) is the form to be used to give notice to the defendant of your intention to make a complaint.
